The Role
We are hiring Full-Time Care Coordinators to support patients with chronic conditions through Chronic Care Management (CCM). This is a phone-based, productivity-driven role where you will spend most of your workday on the phone speaking with patients, coordinating care, and documenting interactions.
This role is ideal for clinical professionals who are comfortable with high call volume , structured schedules, and meeting daily productivity metrics in a remote environment.
Work Schedule & Expectations
Full-time, 40 hours per week
Coverage required Monday–Saturday, 8:00 AM–6:00 PM in the timezone you support.
Fixed schedule assigned (no alternate shifts available)
Expect to spend 7–8 hours per day on the phone
Productivity and quality metrics are required and monitored
What You'll Do
Conduct monthly CCM phone calls with assigned patients
Make 30–50 outbound calls per day
Complete required patient interactions to meet CCM service goals
Educate patients on chronic conditions and care plans
Coordinate care with provider offices, pharmacies, and community resources
Identify gaps in care and social determinants of health
Create and update personalized care plans in the EMR
Accurately document all patient interactions
Maintain HIPAA compliance and patient confidentiality
Requirements
Minimum 1-3 year of experience as a CMA/RMA, LPN, or RN
Active certification or multistate license for LPN/RN
Strong verbal and written communication skills in English
Comfortable being on the phone most of the day
Experience with EMR/EHR systems
Organized, reliable, and able to work independently in a remote setting
Preferred:
Geriatric patient experience
Care coordination or case management experience
Strong verbal and written communication skills in Spanish

This role requires you to think creatively to help patients find resources that address their specific barriers, educate them about their conditions, and coordinate treatment plans with physician offices as needed

You must be comfortable handling a high call volume and meeting standard productivity goals as outlined by your leader.
Responsibilities:
Conducting monthly CCM phone calls with patients, practices, pharmacies, etc. to ensure that your patients are getting what they need between office visits. Expect to spend 7-8 hours on the phone daily.
Creating and/or revising personalized care plans for each patient you call, in coordination with the practices and providers you support.
Identifying social determinants of health, gaps in care, and eligibility for assistance or other referral services for each of your patients. Collaborating with office staff, other community programs, and resources to address all of these effectively.
Empowering patients to take charge of their own wellness and goals in the context of their care plan.
Making approximately 30-50 phone calls daily to yield the 15 patient interactions necessary to complete 20 minutes of CCM service for the month.
Maintaining your patients' privacy, confidentiality, and safety, and adhering to ethical, legal, and accreditation/regulatory standards at all times.
Requirements:
At least 1 year experience as a CMA/RMA, LPN, or RN, with an active accreditation/professional license required.

What You'll Do:
In this role, you'll support chronically ill patients by:
Conducting monthly care management calls
Creating and updating personalized care plans
Identifying health barriers and social needs
Educating and motivating patients
Coordinating with physician offices and resources
Meeting productivity and documentation standards

What We Offer

Compensation:
Training: $21/hr
After training: $16/hr (CMA/RMA) | $18.29/hr (LPN)
Incentives: $800-$1400 monthly (based on performance metrics and productivity)
What We're Looking For
Active CMA/RMA, LPN, or RN (2–3 years' experience)
Spanish Speaker Fluent (Speak, Reading and Writing)
Excellent communication and patient engagement skills
Experience with EMR/EHR and digital tools
Organized, self-driven, and comfortable with high call volume
Geriatrics or care coordination experience is a plus
Technical Needs
Desktop/laptop (i5/Ryzen 5+, 16GB RAM) (no tablets)
2nd monitor (required)
Noise-canceling headset
Reliable wired internet (100 Mbps+ download)
